The multi-disciplinary team focus their treatment plans on independence and community inclusion to improve quality and life and assist with a seamless transition back into the community. Discharge planning begins as soon as an individual is admitted to Seaford Ward. We offer a variety of discharge pathways, including returning to the community, supported accommodation, or residential/nursing care. The team collaborates closely with other providers to ensure a smooth transition at discharge. This collaboration starts at admission and continues as we work with additional professionals once suitable accommodation is identified. Depending on the individual’s needs and the therapeutic support required, we assess that stays may range from 6 months to 2 years.
Service users are encouraged to personalize their bedrooms and are actively involved in weekly community meetings, which also serve to help make the ward more comfortable for everyone. At Seaford Ward, we are proud to offer a service centred around recovery, working collaboratively with service users to identify and support their mental and physical well-being, as well as needs in housing, employment, education and relationships.
